Parental Permission Form

As part of your child’s involvement in Music Mecca, we would like to ask them to take part in a programme evaluation Survey.  The survey will ask about how they found out about the project, in what ways their involvement has had an impact on them, what skills they have learnt from being involved and what they think about the Dry Bar events and workshops.

Participants will be asked to fill out a survey at regular times throughout their involvement in the project so that we can monitor changes over time.  The Survey is online and takes about 10 minutes to complete.

Doing this online survey will cause no risk to your child.    The survey has been designed to protect your child’s privacy.  Participants will be asked for their name and date of birth only to match up their responses over time.  Their personal data will not be used for any other purpose.  Also, no individual child will ever be mentioned by name in a report of the results.

The results of this survey will help us to develop the project and should help similar projects involving children in the future.  We would like all participants to take part in the survey, but the survey is voluntary.  No action will be taken against you, or your child, if your child does not take part.  Participating children can skip any question that they do not wish to answer.  In addition, they may stop participating in the survey at any point should they wish.
